Formed in October 2001 as the only non-profit foundation in Northampton County, PA solely dedicated to providing funding to improve the health and well-being of community residents. As of 2/28/26, the Foundation's assets were $16,900,600.

Honoring the Legacy of Dr. Joanne D’Agostino
The Two Rivers Health & Wellness Foundation recognizes and honors the lasting contributions of Dr. Joanne D’Agostino, whose decades of work in healthcare, nutrition, and community leadership helped strengthen the mission and impact of our Foundation.
Dr. D’Agostino’s distinguished career spanned more than 50 years in nursing, hospital administration, nutrition counseling, writing, and public education. Her commitment to health literacy was well known through her books, editorial work, and her long-running Healthy Heart column in The Express-Times, where she educated the community on heart‑healthy living for more than two decades.
Beyond her professional accomplishments, Joanne served the community through extensive board and service roles. Her time as a Board Member here at Two Rivers Health & Wellness Foundation was marked by thoughtful insight, compassion, and an unwavering dedication to improving the health and well‑being of Northampton County residents. She helped shape key conversations, strengthen strategic direction, and reinforce the Foundation’s commitment to prevention, education, and access to care.
Dr. Joanne D’Agostino’s influence continues to guide our work, and her dedication to community health will remain an integral part of the Foundation’s mission. We are deeply grateful for her service, her leadership, and the indelible mark she left on our organization and community.

The "Eat Real Food" Mobile Market is a combined initiative of the Two Rivers Health and Wellness Foundation and Kellyn Foundation, connecting the "Healthy Lifestyles" education that Kellyn Schools provides children in the classroom to their families and friends in the community. The Mobile Market also provides healthy food access to seniors in many local housing facilities and to residents in neighborhoods without easy access to a grocery store.
The "Eat Real Food" Mobile Market visits multiple sites on a regular weekly Basis. The full schedule of the stops can be found on the Kellyn Foundation website. Each of the weekly site stops includes produce sampling and an educational component, such as nutrition, cooking, exercise, gardening or chronic disease screening.
